Assessment:

 

Comprehensive clinical services to older adults and their families.


 Evaluation and assessment of physical, functional, and social abilities of older adults.

  Offer counseling and emotional support.

Intervention:

  Services available to the older adult experiencing lifestyle adjustment including: loss, retirement, relocation and the aging process.  Health issues (physical and emotional) including: stress, anxiety and depression

Education and counseling to alleviate the strain and stress experienced by adult children, caregivers and family members.


Alzheimer's disease and other dementias, Parkinson's Disease, Multiple Sclerosis, Cardiac, Stroke, Cancer, Diabetes, Orthopedic and other illnesses.

Care Management:

  Coordination of services and ongoing monitoring of home care and placement alternatives.
 

 Assist families with long distance care giving and support.


 Assist with moving an older adult to or from home, a retirement community or nursing home.


Financial management services to assist with monthly household bills, and medical insurance claims.

Resources:

  Knowledge of a wide range of services and community resources including housing alternatives (nursing home and assisted living), rehabilitation programs, hospice and home care alternatives.
  Provide consumer education and advocacy
  Entitlements and Services
  Referrals to Elder Care Attorneys, Physical, Speech and Occupational therapists and support groups.

Speakers Bureau:

  On-site workshops and lectures available for large and small groups individualized to your needs
 

Topics include the aging process, entitlements and benefits, eldercare issues, care giving, housing alternatives, bereavement, community resources and health issues.
